I'm looking for a paper FAA medical form worksheet I can print out, add notes
to, and take with me to the medical for filling out the real form.

The real form is controlled and all the online things I've found, such as
the AOPA turbo medical, want you to do it online, which is NOT what I want.

Go here to fill out the form, print it, and take it with you to the doctor.
This system retains some of your info for your next visit, but I have only
used it a few times not, and don't remember how much.

If you actually submit it on this site, all your doctor will need is the
confirmation number.

If you mean Form 8500-8, AOPA appears to have a scanned-in version in
PDF format here:

Seems stupid doesn't it... why wouldn't it be ok to fill the thing out at
home and bring it in like every other doctor's office in the world.

Anyway, I just sent you a PFD of the form... assuming your email above is
correct you should get it in a minute.

Assuming that this is not your initial medical, get a copy of the last form
you submitted from your AME's office. That, plus whatever changes have
occurred since your last medical, should make a near-perfect template.

That's the same one I was trying to send you.. despite have a sample
watermark, it is blank and should serve your purposes.

That's about a year and a half away.

The issue is at my last medical, the blood pressure was close to, but below
the magic number.

After that I went to the regular doctor, got perscribed an FAA approved
ACE inhibitor to make sure I didn't stroke out, lost 40 pounds, eliminated
a big source of stress (which is a story in itself) and am now down to
marginally high.

By the time the medical comes around, I expect I will have been off the
drugs for a year, have lost another 25 pounds, and be well into the
"normal" range.

The issue is that I will have to document all this crap on the form and
I don't want to do it from memory.

It's a lot of fun to go into a medical (any medical exam, really) with a
change like that behind you. Enjoy. And congrats.

Yeah... you don't want to give the bored bureaucrats the smell of red meat.

I'm sure you know that you should walk into the medical with a
"Hypertension Follow-Up Report" from your treating doctor.

Basically, a signed letter from the Doctor listing all the meds, dosages,
side effects, complications related to hypertension.. AND three blood
pressure readings. The three readings are the big deal.

Don't drink any coffee that day and shoot 10 mg. of Valium directly into
your veins before taking the readings. ;- )

Hey, kudos on the weight loss... 40 pounds could get you a guest appearance
on The Oprah Winfrey Show.

Which would be bad because it doesn't allow the AME to make
"corrections" to your application. Many AME's will say "you may not
want to write that like that; write it this way", etc. If you are
going to an AME who is a regular practice Dr and just does a few FAA
meds on the side to pay for his secretary you should run away. Go to
someone who specializes in FAA medicals and knows what the FAA is
looking for.
