"Tausif Rehman, a Cotton-O'Neil neurosurgeon, was piloting the twin-engine Beechcraft Baron airplane when it left Midway International Airport in Chicago at 10:35 p.m. Oct. 12 en route to the Lawrence Municipal Airport, according to the NTSB report.

Also on board were two passengers: Ali A. Kanchwala, a Cotton-O'Neil pulmonologist, and Kanchwala's wife, Maria Javaid, an interventional cardiologist at Providence Medical Center in Kansas City, Kan.

After takeoff, the Beechcraft rose to an altitude of 2,200 feet above sea level before turning left and beginning a descent, according to the report. The plane then turned to the right and descended 1,500 feet before climbing again and turning left "for about 360 degrees," according to the report. At that time, just five minutes after the plane had taken off, radio contact was lost.

The plane crashed in the 10100 block of S. 86th Court in Palos Hills, less than seven miles southwest of Midway International Airport."
