I'm wondering if the number of multiple postings of the same message
are caused by some kind of system error. Or is it simply caused by
people who post a message, then go to look to see if it came out right,
don't see it, and post it again. I kind of doubt this latter, because
the messages are usually identical. Maybe it's just people
accidentally hitting the send button twice. It may depend on what tool
they are using to create and send the post, as well. I personally use
Google Groups to read and respond to r.a.s. and haven't seen any such
problems.

One cause is that a number of newsreaders (like Thunderbird, which I
use) will detect and display an error after the message has been posted,
but not make it clear that the message has, in fact, already been
posted. Sometimes I forget that it might do this is fix whatever it was
complaining about, then hit send again...

Google
