Lately they've been giving aviation a bad name. At least he wasn't snorting

Question: Has cocaine use ever been implicated in an commercial or air
transport crash?

(I don't think the immediate use would be a problem, but the longer-term
effects on personality and performance habits might....paranoia,
overconfidence, etc.)
