Ok I have a signed bill of sale 8050-2 & a application for regestration 8050-1 now do I send $5.00 for each form or do i just send $5.00 to the faa in OKC, OK little confusing the way the forms read. Do i need to include any thing else with the forms like a letter?

The AIRWORTHINESS CERTIFICATE is a bit worn and stained still readable is there a way to get a new fresh replacement copy of it.
