The D.E. who gave me my checkride (I passed) typed out the form and
handed me the temp license and told me very specifically how long it
might take to get the permanent card. He then counted out the months
and told me to get back to him at such and so a date if I had not
gotten it by then. I forget exactly how long you have before the temp
license expires but 4 months may be close.

I got the plastic card about three months later.

Nasir,

Temp. Airman's Certificate expires 120 days from date of issue. Your
DPE should have made a copy of all the paperwork before sending the
originals in. Prior to the "date of expiration", contact DPE and let
him/her know you did not get your permanent certificate. (I personally
would start calling after 110 days depending on how hard it is to get
ahold of him/her.) Your DPE can print you another temp. certificate
and either start back tracking or recreate paperwork to send in again.
Either way, a retest will not be needed.
