In USAF it was standard procedure to open the refueling door prior to
shutdown, on flights where we had refueled, so the crewchief could inspect
and lube it. Perhaps the GAF uses a similar procedure, but opens the door
in a different place in the checklist.

Also could have been a malfunction during the refueling and the checklist
calls for not moving the receptacle until inspected.

I have noticed that when a German F-4F Phantom is landing, the refueling
"door" on top of the aircraft is open. Does anyone know why this is? I
haven't noticed it on Phantoms from other contrys.
