How about the DG-600 prototype?


Aileron flutter far over Vne because Wilhelm Dirks removed all
flutter-damping devices for this test flight.
One aileron started to flutter and a wing broke off, Wilhem Bailed out
at high altitude (about 10.000 ft iirc).
This happened ten miles from where I live.
