Could also be a technical issue with the trim system. There have been failures in the past where the trim indicator in the cockpit did not accurately represent the actual trim position of the elevators/horizontal stabilizer/trim tabs and the aircraft subsequently took off with nearly full nose-up trim.

Tough video to watch...

Looks to me like these guys did a great job getting it back level. Maybe with another 500-1000 feet they could've slammed it on? I realize with loose cargo it may not have mattered.

Apparently the former: an MRAP (Mine Resistant Ambush Protected)
truck. These things are meant to drive right through a hot combat
area, screw the mines and IEDs -- they run 28,000-40,000 pounds.

Presumably it broke loose on rotation.

From the weekly union email:

Tragedy struck this week, as Flight NCR102, a National Airlines
B747-400 cargo plane, crashed on takeoff from Bagram Airbase in
Afghanistan on April 29. There were seven crewmembers on board.
Unfortunately, none survived.

This event hit close to home – Rinku Summan, a furloughed Mesa pilot
based in ORD, was one of the crewmembers on board. He is survived by
his wife, Rajnit, who is also a former Mesa pilot, and their son,
Adarsh. Our thoughts and prayers are with them, as well as their
friends, and the family members and friends of all those on board.
