IIRC, there was an article recently in one of the aviation pubs (AOPA Pilot, I think) where a pilot flew a doctor and nurse to a crash site for no pay, and wound up getting written up by the FAA. Their view was that, even though the pilot didn't charge anything for the flight, the doctor and nurse WERE being paid, and therefore, it was deemed a commercial/135 operation. OK.
