Default "Air Crash Investigation"

Everybody, do yourself a favor and watch this excellent series on
YouTube. Each episode runs about 47 minutes. There are about 40
episodes. They mostly deal with airline crashes but gliders are very
similar. The cause of almost all of them is pilot error - sometimes
painful to watch errors. It will give you a new perspective on
accident causes.

I started to DVR "Air Disasters" on the Smithsonian channel for same reason.

Looking at episode lists. It is the same episodes under a different title. The Smithsonian channel is in season 2 a season behind The National Geographic Channel which is in season 3.

It never seems to be just one thing that causes an accident. Its
always a combination of things.
