Default Best/Worst Safety record/practices by country, region, airport, demographics

Pilots make mistakes and die everywhere, but which places/demographics do better/worse than others?

I know that no one has any across the board hours_flown:fatalities metrics, but I wonder if anecdotes and impressions have some value.
