Default Advanced rating training as Wings credit

You must attend a safety meeting and complete the
appropriate training. Then submit the forms to the FAA
FSDO.

At home I have some signed off seminar attendance forms that include spaces for date / details / CFI info for the flight portion. Is that the form?

Yes, that's the form. It's not well designed (makes no provision for
instruction that is not exactly one hour, or from different instructors,
but the FAA doesn't seem to care if you do it that way.

Just be sure to put the flights and the seminar in your
logbook too. Be sure to have your CFI sign the logbook and
the form and that the form was signed by the seminar sponsor
also. It is a good idea to indicate the flights that
constitute the Wings training.
