I have a feeling that B-24 did a head stand after some variety of
nose-wheel failure.

Yes, I believe most and perhaps all aboard survived this one as it is
clearly a very survivable accident.

The U.S. National Archives provides a description for this photo:
Crashed on take off from San Giovanni Field,Italy on April 12,1945 killing
6 of the crew.
B-24M-5-FO Liberator - s/n 44-50468.
740th Bomb Squadron, 455th Bomb Group, 15th Air Force.


B-24M-5-FO S/N 44-50468 belonging to 455th Bomb Group 740th Bomber
Squadron, 15th Air Force. "Supposedly the result of inadvertent application
of brakes during takeoff run, this wartime accident killed six crewmen
almost instantly." occurred at San Giovanni April 12, 1945. Pilot listed
as James T. Harris.
